Enrollments are by appointment only. To make an appointment, please contact the counseling office at 770-819-2611 ext. 224. Required documents for enrollment: *Georgia Certificate of Immunization
(form 3231) *Georgia Eye, Ear, & Dental
(form 3300) *Proof of Birth *2 Proofs of Residency *Social Security Card *Transcript/Withdrawal form from prior school attended
